Field Trip Information & DWS Liability Waiver
This document will be shared by through the DWS Blackbaud Portal prior to your student's trip and is specific to each field trip.
NOTE: A new liability waiver needs to be signed and submitted for each field trip.
Volunteer Agreement
All chaperones need to complete a Volunteer Agreement form and submit it to the office prior to their first field trip of the current school year. Once completed, the form is good for the full school year. A new form must be turned into the office every school year.
LiveScan Request Form
All overnight field trip chaperones must undergo a “supervisory level” background check, administered through our Business Manager. This will entail providing fingerprints through a LiveScan system, using the LiveScan request form provided by the Business Office. This will provide the school with updates through the FBI and Department of Justice database system for as long as you remain an active volunteer at the school. We recommend class parents and any guardians expecting to volunteer/chaperone for multiple events complete a LiveScan. The LiveScan costs $67* (subject to change) and is good for the entire time you have children attending DWS. Please print and fill out the request form and take it to your nearest LiveScan location (click here for more info).

Automobile Use Form
All drivers need to complete a Automobile Use Form and submit it to the office prior to their first field trip of the current school year. Once completed, the form is good for the full school year. A new form must be turned into the office every school year. The form includes our automobile use policies and requires you to include and verify your auto insurance information.
DMV Record
In addition to the Automobile use form, you will need to submit a copy of your driving record. You can request a copy of your record online from the DMV (click HERE). The process takes a few minutes and costs $2.
